The LCSW will provide direct practice mental health services such as clinical assessments and evaluations, treatment planning, psychotherapy, crisis intervention, clinical case management, and internal and external care coordination.\u00a0The LCSW may work with individuals, families, or groups.<\/p>\n<p>The LCSW is responsible for professionally applying social work methods and theory to the prevention, assessment, diagnosis, and treatment of psychosocial dysfunction, emotional and behavioral issues, and psychiatric disorders.\u00a0As an employee of La Maestra Community Health Centers and a licensed professional, the LCSW must always act in accordance with La Maestra\u2019s policies, procedures, and protocols as well as those established by the California Board of Behavioral Sciences. It is essential for the LCSW to have considerable knowledge of mental health services, dynamics of human behavior and development, characteristics of the population served, and corollary treatment modalities. The LCSW will be expected to work in a medical setting and interact collaboratively with medical teams.<\/p>\n<p><strong>Responsibilities<\/strong>:<\/p>\n<ul>\n<li>Conduct biopsychosocial evaluations on patients and\/or families to assess the extent to which health-related, psychological, social and spiritual factors may be contributing to presenting problems.<\/li>\n<li>Utilize validated tools, methods and approaches to assess, diagnose, and inform treatment.<\/li>\n<li>Prepare individualized care plans (IAP) to address the unique biopsychosocial needs of patients, in objective and measurable ways, within 30 days of initial patient contact.<\/li>\n<li>Provide ongoing treatment services, monitor progress of treatment, refer for additional services, re-evaluate, and terminate services as necessary.<\/li>\n<li>Facilitate individual, group, and couple therapy for patients and\/or families using evidence-based practices and within the providers scope of training and practice.<\/li>\n<li>Meet with family members and significant others to gain collateral and enlist their support in carrying out the patient\u2019s treatment.<\/li>\n<li>Provide crisis evaluation, clinical case management, and other supportive services.\u00a0Refer, link and facilitate access to appropriate community entities.<\/li>\n<li>Provide crisis intervention, emergency referrals, and urgent care coordination as needed.<\/li>\n<li>Work collaboratively with other treatment team members (i.e. psychiatrists, primary care physicians, case managers, etc.) to coordinate and provide integrated physical health and behavioral health services to patients.<\/li>\n<li>Use La Maestra-endorsed and other validated tools to complete and document intake assessments, reassessments, IAPs, progress notes and discharge summaries.\u00a0Track and ensure all assessments are up to date.<\/li>\n<li>Generally, complete documentation within 2 business days of services rendered, with consideration for expedited or delayed reporting factors.<\/li>\n<li>Assist with maintaining up to date medical records to include contact information, demographics and funding information. Report accurately the nature and length of all services rendered.<\/li>\n<li>Remain familiarized and up to date with current behavioral health methods and practices.<\/li>\n<li>Be knowledgeable about and uphold confidentiality and privacy practices.<\/li>\n<li>Maintain productivity standards.<\/li>\n<li>Contribute to a warm, welcoming and professional environment for patients, coworkers, partner agencies and the general public.<\/li>\n<\/ul>\n<p><strong>Job Requirements<\/strong>:<\/p>\n<p><strong>Education:\u00a0<\/strong>Master\u2019s degree from an accredited school of social work.<\/p>\n<p><strong>Certification required<\/strong>:\u00a0Active Licensed Clinical Social Worker in the state of California.<\/p>\n<p><strong>Experience<\/strong>: Experience working in a community health or mental health setting.<\/p>\n<p><strong>Verbal and Written Skills to perform the job:<\/strong>\u00a0Bilingual preferred. Familiarity with basic word processing, spreadsheet, and database applications.\u00a0Accurate keyboarding skills.<\/p>\n<p><strong>Technical knowledge and skills required to perform the job<\/strong>:\u00a0Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups.\u00a0Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams.\u00a0Skill in analyzing situations and making timely decisions.<\/p>\n<p><strong>Physical and Mental Requirements:\u00a0<\/strong>Work involves sitting, talking, hearing, using hands to handle, feel or operate objects, tools, or controls, and reaching with hands and arms.
